Ward 777 takes players on a disturbing journey inside a scary, death-like asylum where the storyline keeps mixing and blending, and, at the same time, the players go through the dreadful insane hospital with their lantern and have to deal with a horror of mind breaking their sanity. The game is centered on exploring the constantly changing corridors and haunted rooms of the hospital, with the player only holding a dimly lit lantern. This light can be used both as a tool to reveal some of the spectral anomalies hidden in the dark and as a lifeline that, for a short period, holds back the surrounding shadows. However, if the light is used too much, it will be a way of the “Caretaker,” a sound and light-hunting entity that is the most terrible of all, getting to you.

The main activities of the game are supported by a very complicated “Sanity System” where your stress level influences your understanding of the surroundings: the barely audible speech changes into screams; the walls you see start to show signs of bleeding; what looked like a kind person at first might now seem a monster that even attack you; and so on, compelling the player to make quick choices of either fighting back or helping.

The game is turned forward by finding patient logs and medical tapes, which piece together the hospital’s dark history. At the same time, every time the player gathers a new piece, he loses a little of his sanity, thus creating a risk-reward loop that is very tense and in which the player has to decide whether to go for the truth or fall into madness.

There is no fighting as in other games, and if you want to survive, you have to use stealth, environmental interaction (for example, opening doors or solving eerie puzzles), and taking good care of your fragile mental state. Every new encounter will be a very difficult moment, as your heart will pound. You will have to trust your gut and your nerves to deal with it in a place where the biggest monster can be the one that you are slowly waking up inside you.
