Players taking part in Worshippers of Cthulhu are thrown into a world of darkness: half cults and eldritch horror, half RPG role-playing with a touch of Lovecraft mythos to boot, and set in the 1920s. It is a 2D cultist management game, which means your main task is to become a cult initiator and call Cthulhu with the help of various rituals, knowledge, and manipulation with the locals. The game shares some elements with stealth strategy game and horror as you move through the creepy foggy towns and cities to find rare pieces and uncover secret texts scattered in abandoned and haunted libraries, churches, and caves. The only problem is numerous alert officials, and civilians will notify the authorities if they think you are involved in some form of evil activity. With each level up, the character’s abilities are more perverted than in classic horror, like mind control, becoming a shadow, and tricking enemies into not capturing you.

But Worshippers of Cthulhu isn’t just for gathering power; it’s also for keeping one’s sanity afresh. The first task and any encounter with the occult, any sighting of a horrific creature, and any reading from the old tome all erode the character’s sanity. The game adds the known sanity meter, which changes at every meeting: each time, you stop seeing the world entirely and are on the edge of crazy. At lower levels of sanity, the player gets disturbing vision effects, distortion of sounds heard, and sees illusion, which is hard to distinguish from reality.

Regaining, or losing, sanity is another central element of gameplay; being overwhelmed might make your character clatter in terror or paranoia, messing up your stealth-based missions, or running around, spherically waving, perhaps drawing the attention of the police to you. Every decision takes you farther down a grim branch of an expanding story in which submissiveness to Cthulhu’s beckon could be accompanied by incredible capability or a suicidal obsession.
