Pathologic 3 immerses gamers in a chilling, systemic survival story of a plague-stricken, sentient town, which is a biological and social experiment running unconsciously. The gameplay is a struggle of the last days to manage limited resources, solve a metaphysical mystery, and make allegiances while being infected and socially disintegrated. Each step is a significant compromise in a delicate environment: spending time negotiating with distrustful NPCs in a complicated, dialogue-influenced market means less time for searching for very necessary antibiotics, sterilizing polluted water, or exploring the disturbing, player-initiated backstory that indicates the town itself as a living, evil being. The plague acts in a horror-reality way, propagating itself through areas which you can isolate or let go, changing according to the players’ collective decisions, and revealing symptoms which you have to discover and treat with scarce, mainly trial, medical resources. Survival depends on the player being able to weave his way through a web of survival layers, managing hunger, fatigue, immunity, and notoriety while following one of the several intertwined character storylines, each of them highlighting the crisis from a different philosophical perspective, but, in fact, none of them revealing the full truth. That results in an unbearable, deeply immersive experience in which there are no winners, only lessened tragedies and, ultimately, the real aim becomes not to “win” but to figure out what kind of moral sacrifices you are willing to make to live another day in a world that is actively trying to break down your body, mind, and humanity.
