Mafia: The Old Country takes players back to the harsh, sun-drenched landscapes of Sicily in the early 1900s, where honor, loyalty, and violence are part of every decision. It is a mix of cinematic storytelling and authentic period combat with an immersive open-world feeling. At the heart of the game is the “respect economy”, irrespective of the type of the mission: a hit, offering a favor to a local don, or a tile dispute, each will give you rispetto in exchange for which you can get new weapons, allies, and story branches, but losing it via cowardice or betrayal will result in sealing off whole questlines. Combat is slow and merciless: for melee attacks, you have a directional parry system (high, mid, low). On the other hand, weapons like the Lupara shotgun and Carcano rifle come with realistic reload animations and very strong recoil, hence you need to consider tactical waiting and cover working. The “omertà system” is a stand-in for dialogue wheels; communication is done through silence, gestures, or cryptic phrases, and your reputation with each family determines whether NPCs will offer help, sell information, or set an ambush for you. Exploration covers the whole island of Sicily, from the crowded streets of Palermo to the small hilltop villages and olive groves used as hideouts. At the same time, the dynamic weather and time of day changes affect NPC patrols and mission availability. “Dual-timeline” narrative enables you to see Enzo’s development in the 1900s and the main story in the 1920s, with different kinds of weapons and societal norms in each. The “vendetta tracker” records every family you’ve offended; unresolved grievances will result in you being hunted when you are just roaming around the open world. “Horse-and-cart” mode of transport brings in realistic stamina and road conditions, yet you can also catch trains, lift luxury cars, or take boats along the coast to get to islands.
